Deep burns destroy hair follicles within the affected skin, leaving permanent scarring alopecia. Common sites include the scalp, eyebrows, beard and chest. The scar's quality matters: thin, pliable scar with a good blood supply tolerates hair transplantation, while thick, tight or radiated scars may require scalp scar hair restoration through tissue expansion or flap reconstruction first.

Initial assessment maps the area of hair loss, scar quality, donor hair availability and surrounding skin laxity. We photograph the scalp, design a hairline if relevant and discuss what density is realistic. Patients with extensive burns may benefit from staged plans combining smaller hair transplant on burn scars with larger tissue expansion for surrounding bald areas.

Hair transplantation in burn scars uses follicular unit extraction (FUE) most often, since it allows careful placement and can be repeated. Graft survival is lower than in non-scarred scalp because the scar's blood supply is limited, so we plan multiple smaller sessions rather than one large one. This approach gives steadier hair regrowth after burn injury and predictable cosmetic results.

Tissue expansion is suitable for moderate to large bald areas with healthy hair-bearing skin nearby. A silicone expander is placed under intact scalp and gradually inflated over several weeks. The expanded skin then covers the resected scar, replacing it with hair-bearing tissue. This is one of the most powerful tools for cosmetic hair restoration after extensive burns.

Flap reconstruction is reserved for very large areas or scars over compromised tissue. Local rotation flaps and selected free flaps may be used. Eyebrow and beard reconstruction often combines micro-grafting with careful design of hair direction and density. Each plan is built around the patient's age, hair texture, expectations and how visible the area is in daily life.

Postoperative care includes gentle cleansing, sun protection and avoidance of tight headwear during early healing. Hair transplant graft regrowth typically appears between three and six months, with full results at twelve to fifteen months. Tissue expansion takes around two to four months from start to final reconstruction. Follow-ups continue at regular intervals to monitor progress and plan further sessions.
