About Bedsore over Sacrum (Lower Back)

The sacrum is the most common site for pressure sores because it bears the body's weight when patients lie on their back. Prolonged immobility, paralysis, severe illness, ICU admission and ventilator support all increase the risk. Bedsore treatment lower back planning at Kayakriti starts by mapping the wound, the surrounding skin and other vulnerable areas, since sacral ulcers often coexist with hip and heel sores.

Sacral ulcers progress through four categories. Stage I shows persistent redness without skin loss, while stage II involves shallow open wounds or blisters. Stage III extends into fat, and stage IV reaches muscle or bone. Patients and families are encouraged to consult a plastic surgeon early, because sacral ulcer treatment is far easier and less invasive when addressed before deep tissue damage develops.

Pressure sore sacrum treatment for Category I and II wounds is usually conservative. We provide site-specific dressings, oral antibiotics if there is local infection, and clear advice on offloading, hygiene and nutrition. Reviews are weekly at first and then once or twice monthly for three months after healing. A dedicated air mattress and frequent turning remain central to wound care bedsore protocols.

Category III and IV sacral ulcers require admission for thorough evaluation. We assess nutrition, anaemia, diabetes, kidney function and overall fitness, and obtain pre-anaesthetic clearance from physicians, cardiologists and neurologists where needed. Once stable, Dr. Agarwal plans surgical debridement and flap cover, most often using gluteal rotation or V-Y flaps that bring well-vascularised tissue over the sacrum.

Surgery is performed under general or regional anaesthesia and includes removal of all dead tissue, control of infection and reliable flap cover. A suction drain sits beneath the flap to prevent fluid collection. In selected complex wounds, two-stage procedures are used so that infection is controlled before final reconstruction. Pain control, antibiotics and strict positioning protect the repair through early healing.

Discharge planning is detailed: air mattress use, two-hourly turning, skin checks, dressing technique and nutrition advice are all covered. Follow-ups are scheduled at one week and then once or twice monthly for three months as part of our chronic wound treatment in Lucknow. Our aim is durable healing, low recurrence risk and a confident family team caring for the patient at home.

Common questions

Frequently asked questions

How serious is a sacral bedsore?
Severity depends on the stage. Early redness or shallow wounds often heal with good dressings and pressure relief. Deeper ulcers can expose muscle or bone, raise infection risk and slow recovery from the underlying illness. We recommend prompt assessment so you understand the stage and can begin appropriate treatment without delay.
Can a sacral pressure sore heal without surgery?
Yes, when caught early. Stage I and II ulcers usually respond to specialised dressings, infection control, optimised nutrition and consistent offloading. Surgery is reserved for stage III and IV ulcers, where flap reconstruction provides reliable cover. Dr. Agarwal will recommend the least invasive option that fits your situation.
Who performs the surgery and what is their experience?
Dr. Amit Agarwal, FRCS Edinburgh and American Board of Plastic Surgery, leads sacral ulcer reconstruction at Kayakriti. He has long experience with gluteal flap surgery for chronic wounds, and works with a dedicated nursing team that supports families through admission, surgery and structured follow-up after discharge.
What does sacral bedsore treatment cost in Lucknow?
Costs depend on stage, surgical technique, anaesthesia and length of stay. Conservative dressing care is much less expensive than flap reconstruction. We provide a clear, itemised estimate after the initial assessment, including imaging, theatre, ward stay and follow-up reviews, so families can plan finances confidently.
How can we prevent the sacral bedsore from coming back?
Use a quality air mattress, turn the patient every two hours, and keep skin clean, dry and free from moisture damage. Optimise protein intake, control diabetes and treat incontinence early. Our team trains relatives in skin checks and dressing technique, with scheduled video and in-person reviews after discharge.
